Treating newborns with hypoglycemia requires a rapid, safe way to raise blood glucose. A bolus of dextrose at the right concentration achieves this: 10% dextrose given at 5 mL/kg IV. This delivers enough glucose quickly to lift levels without overwhelming the neonate’s small veins with overly concentrated solution.

Why this concentration and dose is preferred: 10% dextrose provides rapid glucose delivery, essential in a hypoglycemic newborn. Using a higher concentration, like 50% or 25%, increases osmolar load and risks vein irritation or tissue injury in such delicate vessels. D5W (5% dextrose in water) does not raise blood glucose quickly enough and can dilute circulating glucose if given as a bolus, plus it can cause other fluid balance issues. The 5 mL/kg dose of D10 balances quick correction with safety for neonatal vasculature.
